Our Changing Tides Program is there to support you and your family through the mental, physical, and emotional changes that may accompany life-limiting illness.

We are there every step of the way to offer guided, non-medical, whole-person care that’s tailored to your specific needs and wishes. Our program is provided by a team of trained volunteers and professionals.

Non-medical, client-focused care includes:

  • Assessment by a Licensed Clinical Social Worker

  • Care Coordination with your primary care provider, palliative nurse and other professionals to ensure smooth and effective communication and care.

  • Navigation & Referrals to appropriate community services such as PCA services, lifeline, senior meals, and assistive equipment. We also provide assistance with navigating financial, legal or planning issues as they relate to hospice and palliative care.

  • Respite Care allows short-term relief to family and caregivers. Our volunteers’ care and support makes caregivers’ work more sustainable.

  • Volunteer Companioning includes empathetic listening, reading / writing, going for walks or drives, light housekeeping, friendship, etc. This care is offered by trained volunteers ready to walk this journey with you.

  • Indirect Care includes running errands, meal preparation, and handyman services. These tasks can be arranged within your Plan of Care.

  • Emotional & Spiritual Support is provided to clients and family members by qualified individuals as desired. Sometimes what’s needed most is to simply to hold space through these challenging times.
